- Accurate, timely documentation — Quarterly reports are provided to Support Coordinators and decision-makers, covering medical appointments and outcomes, so all stakeholders stay informed.

- Initial Meeting Our Admissions Team meets with the individual and their family to understand preferences, needs, and desired outcomes.
- Provider Matching We identify and propose one or more provider homes that align with the individual’s needs and preferences.
- Home Tours The individual and family tour the selected provider homes, meet the provider, and learn about the service environment.
- Trial Visit A short stay (trial visit) is scheduled so the individual and family can evaluate the fit in a living-community setting.
- Service Plan Development & Start DateOnce the fit is confirmed, we collaboratively develop the detailed Service Plan and set a start date. Note: This process is flexible and can be expedited to meet the individual’s timeline.
